Lincoln County Health Department is pleased to share the 2023-2028 Lincoln County Health Assessment and Improvement Plan.
This plan identifies health priorities and provides a roadmap for partners and community members on how we can work together to create a healthier Lincoln County.

Lincoln County Health Priorities
Live Well Lincoln’s mission is to promote partnerships within the community to improve health through advocacy, prevention, and implementation of best practices. The Live Well Lincoln Leadership Team includes: Lincoln County Health Department, Aspirus Health, Aging & Disability Resource Center of Central Wisconsin, and Lincoln County Department of Social Services.
